At Evolve your child can have an autism and/or ADHD assessment through the NHS Right to Choose route. The only way to be referred for this route is through your GP. It may be helpful to download this letter to take to your GP, however the GP does not need a copy of this letter to refer your child.
We also offer autism and ADHD assessments through the privately funded route. If you would like to find out more about the costings of these please contact us.

We offer autism assessments which are completely online or a mixture of both online and in person. Please click the arrow below if you would like to find out what your child may be offered. ADHD assessments are only a mixture of in person and online, we do not offer online only for ADHD appointments.

Based on your answers, your child is eligible for an autism digital assessment which is all online.

If you are wanting an NHS Right to Choose assessment, please request a referral from your GP. It may be helpful to give them this letter. If you would like a private assessment, please contact us for registration forms.

Once you have registered with us, we will be contacting you again with these questions and will confirm the best assessment route for your child. If your circumstances have changed it may be that an in person assessment is more appropriate for you.
